What Should Be Reported Here?
This form is for issues the HOA is responsible for maintaining:
Common Areas
- Walkways, breezeways, and stairways
- Landscaping and irrigation
- Parking lot issues (potholes, striping, lighting)
- Pool area and gazebo
- Exterior lighting
- Trash and recycling areas
Building Exterior
- Roof leaks affecting common elements
- Siding, fascia, or paint deterioration
- Gutters and downspouts
- Exterior doors, locks, or entry hardware
- Foundation or structural concerns
What About Issues Inside My Unit?
Repairs inside your unit (appliances, interior plumbing fixtures, flooring, etc.) are generally the homeowner’s responsibility. If you’re unsure whether an issue falls under the HOA or the homeowner, check your CC&Rs in the Document Library or call VMS Management at 972-726-9000 — they’re happy to help clarify.
Priority Guide
Select the priority that best matches your situation:
- Emergency: Active water intrusion, gas odor, fire damage, or safety threat. Call 972-726-9000 instead of using this form.
- Urgent: Issue that could cause further damage if not addressed soon.
- Normal: Standard maintenance that can be scheduled within a few business days.
- Low: Cosmetic or minor enhancement request.

What Happens After You Submit?
- Confirmation email — you’ll receive one right away.
- Assessment — VMS Management reviews the request and determines priority and scope.
- Scheduling — a vendor or maintenance crew is dispatched based on priority and availability.
- Resolution — once the work is complete, you’ll be notified.
